Enrolment Form
To enrol your child with ICCF Leicester you will need to fill out the following form.
Instructions:
1. Ctrl + Left click on the thumbnail image below, you will be directed to ICCF Picasa Web Albums.
2. Left click Download located near the the top left hand corner.
3. Right click to "save image as" the Enrolment Form to Desktop on your computer.
4. Locate the saved file on your Desktop.
5. Print out, fill in and bring it with you to training.
